Overview

Celecoxib, marketed as Celebrex, is a selective cyclooxygenase-2 (COX-2) inhibitor developed to relieve inflammation and pain with less gastric irritation than older non-selective NSAIDs. Approved in 1998, it became one of the first COX-2–specific anti-inflammatory drugs. Dosage & Administration

Typical adult dosage: 200 mg daily or 100 mg twice daily for osteoarthritis; 200–400 mg daily for rheumatoid arthritis; 400 mg once, then 200 mg if needed for acute pain. Take with or without food; swallowing capsules whole is recommended. The lowest effective dose for the shortest duration should be used.

Most frequent reactions: dyspepsia, headache, hypertension, and upper-respiratory symptoms. Serious but rare events include gastrointestinal bleeding, myocardial infarction, and stroke. Celecoxib is contraindicated for patients allergic to sulfonamides or with recent coronary bypass surgery.

Patient Considerations

Patients with cardiovascular disease should use the smallest possible dose and duration. Concurrent alcohol or corticosteroid use heightens GI risk. Kidney-impaired or elderly individuals need monitoring.
