Overview

Promethazine, marketed as Phenergan, is a first-generation antihistamine of the phenothiazine class with strong sedative and antiemetic properties. It blocks H₁-histamine receptors and also exhibits anticholinergic and dopaminergic antagonism.

Uses

Indicated for allergy symptoms, motion sickness, nausea and vomiting, sedation, and adjunctive treatment of postoperative pain. Also used as a nighttime sleep aid.

Dosage & Administration

Oral dose: 12.5–25 mg every 4–6 hours as needed; for motion sickness, 25 mg before travel. Parenteral and suppository forms are available. Children under two years should not receive it due to respiratory-depression risk.

Side Effects & Warnings

Drowsiness, dry mouth, blurred vision, and constipation are common. Serious reactions: respiratory depression, neuroleptic-malignant-like syndrome, and tissue necrosis with extravasation (IV). Contraindicated with other CNS depressants or MAOIs.

Avoid driving, alcohol, or sedatives. Use only short-term for sleep or nausea control.
